Trochanteric abscess

An abscess that develops at the Shangjiao point of the hip is called trochanteric abscess, also known as bone adherent abscess and shrinkage foot abscess. It is equivalent to acute purulent hip arthritis in modern medicine. In traditional Chinese medicine, the abscess that grows at the knee is called疵疽, the one at the ankle is called足踝疽, the one at the shoulder is called肩中疽, the one at the elbow is called肘疽, and the one at the wrist is called兑疽. Although the names are different, they all belong to acute purulent diseases between joints.

What are the causes of Huangtiao abscess

2. The etiology and pathogenesis of this disease are basically the same as those of附骨疽 (Fu Gudan). In the early stage, there is aversion to cold and fever, the hip joint area is swelling and slightly painful, the skin color does not change, followed by increased pain, difficulty in bending the waist, the buttocks gradually protrude outward, and the thigh slightly outwardly rotates; about one to three months later, the skin becomes slightly red, persistent fever indicates that internal pus has formed. After ulceration, the pus is clear and thin, not easy to heal. Improper treatment or continued development may result in disability of the lower limb on the affected side. Similar to hip joint tuberculosis. This disease can also be caused by nearby trauma or by direct spread from Fu Gudan. The clinical characteristics are that it is prone to occur in children, males are more than females, local swelling and pain, affecting the range of joint movement, difficult to recover after ulceration, and prone to disability.. What complications can Huangtiao abscess lead to

If Huangtiao abscess is not treated in time, there is a possibility of bone necrosis. The patient may experience stiff and damaged limbs, dislocation, and even disability. The treatment of Huangtiao abscess focuses on clearing heat and detoxifying, and harmonizing the nourishment to transform dampness.

3. What are the typical symptoms of Huangtiao abscess

The clinical stages and symptoms of Huangtiao abscess are as follows:
  1. In the early stage, there is aversion to cold, fever, hidden pain in the hip joint area, unchanged skin color, followed by increased pain, inability to bend the waist, prominence of the buttocks, and slight outward rotation of the feet.
  2. In the middle stage, the skin is hot, slightly red, painful, and the swelling extends from the waist and hip to the thigh. Persistent fever with fluctuation upon palpation indicates that internal pus has formed. The suppurative stage is between 1 to 3 months after the onset of the disease.
  3. After ulceration, yellow, thick pus is excreted. If bone damage occurs, the wound is not easy to heal, and most patients become disabled after healing.

4. How to prevent Huangtiao abscess

Huangtiao abscess is equivalent to suppurative hip arthritis. The principles of prevention for suppurative arthritis are early diagnosis, timely and correct treatment, preservation of life, and maximum preservation of joint function. Joint activity and functional exercise should be controlled, and muscle contraction exercise should be started as soon as the local inflammation subsides if there are no adverse reactions. This can prevent joint adhesion and help restore joint function. However, attention should be paid to the local inflammatory condition, and activity should not be too early or too frequent to avoid the spread or recurrence of inflammation.

5. What laboratory tests are needed for the diagnosis of Huangtiao abscess

This disease is equivalent to acute suppurative hip arthritis in modern medicine. The diagnostic methods, in addition to traditional Chinese medicine examination, can also refer to the diagnostic methods of acute suppurative hip arthritis for confirmation.

The diagnosis of this disease is based on medical history, clinical symptoms and signs. In patients suspected of having hemolytic suppurative arthritis, blood and joint fluid bacterial culture and drug sensitivity test should be performed. X-ray examination is not very helpful in the early stage, only joint swelling is seen; in the later stage, there may be decalcification of bone, due to cartilage and bone destruction, leading to narrowing of the joint space. In the late stage, joint ossification or fibrous stiffness and deformity may occur, with new bone hyperplasia, but the formation of dead bone is less common. Suppurative arthritis should be distinguished from acute suppurative osteomyelitis, rheumatoid arthritis, tuberculous arthritis, and rheumatoid arthritis.

6. Dietary taboos for patients with Huangtiao abscess

The treatment of Huangtiao abscess focuses on removing dampness and expelling cold. The following dietary remedies have a good effect on assisting in the treatment of Huangtiao abscess: ① Wu Shen Soup added with Cangzhu (Atractylodes macrocephala) and Huangbai (Phellodendron amurense). ② Bao Yang Huan Wu Wu Soup added with Shenjincao (Ligusticum chuanxiong), Ningwuteng (Sambucus williamsii), and Jixueteng (Spatholobus suberectus).
