Intussusception syndrome

Intussusception syndrome refers to non-specific edema of the ileocecal valve caused by various reasons. Clinical manifestations include recurrent diarrhea, right lower quadrant pain, and weight loss. It is more common in young and middle-aged males and overweight females. If the ileocecal valve and the protruding ileal mucosa together prolapse into the cecum, it can be called intussusception syndrome, also known as ileocecal hyperplasia syndrome.

1. What are the causes of intussusception syndrome

The occurrence of intussusception syndrome is related to edema, ileocecal tumors, parasitic infections, and other factors. The specific etiology is described as follows.

  1. Edema: idiopathic or traumatic.

  2. The ileum mucosa herniates or prolapses into the colon.

  3. Submucosal fat accumulation.

  4. Benign and malignant tumors in the ileocecal region.

  5. Inflammation involving the ileocecal valve: parasitic (ameba), non-specific (Crohn's disease).

2. What complications are easy to cause intussusception syndrome

The main symptoms of intussusception syndrome are recurrent diarrhea, right lower quadrant pain, and weight loss. Intussusception syndrome may be complicated by intestinal stricture, intestinal obstruction, and massive rectal bleeding.

3. What are the typical symptoms of intussusception syndrome

The main symptoms of intussusception syndrome are recurrent diarrhea, right lower quadrant pain, and weight loss. There may also be symptoms of disturbed digestive function such as decreased appetite and abdominal distension. There may be tenderness in the right lower quadrant, but no rebound tenderness or muscle tension. Due to the prolapse of the ileocecal valve, diarrhea and constipation often alternate. A mass may be palpable in the right lower quadrant, or accompanied by varying degrees of rectal bleeding.

4. How to prevent intussusception syndrome

The intussusception syndrome is mainly caused by congenital abnormalities in the ileocecal valve structure, which is difficult to prevent. Early detection, early diagnosis, and early treatment are the key to the prevention and treatment of the disease. When benign or malignant tumors involving the ileocecal valve and inflammation are found, active treatment should be carried out to avoid the occurrence of the disease.

5. What laboratory tests are needed for ileocecal valve syndrome

The examination of ileocecal valve syndrome includes blood tests, barium enema examination, and fiberoptic colonoscopy, etc., and the specific examination methods are described as follows.

  1. Blood test

White blood cell count is basically normal.

  2. Barium enema examination

Typical filling defects can be seen in the ileocecal valve area,呈 rose-like, cap-shaped, mushroom-like, umbrella-like, etc., with local tenderness. The defect in the frontal image is smooth and round.

  3. Fiberoptic colonoscopy

An important examination method. Insertion into the ileocecal region allows observation of the ileocecal valve morphology under direct vision.

6. Dietary taboos for patients with ileocecal valve syndrome

Patients with ileocecal valve syndrome should eat foods with the effect of moistening intestines and defecation; they should eat foods with high fiber content; they should eat foods with high protein content. They should avoid eating high residue foods; they should avoid eating hard and difficult-to-digest foods; they should avoid eating spicy and irritating foods. The specific dietary precautions are described as follows.

  1. When the intestinal function is initially restored, low-protein, low-fat liquid foods such as thin rice soup, thin lotus root powder, fruit juice water, vitamin sugar water, carrot water, etc., should be selected, 50-100ml each time, 3-6 times a day.

  2. Further recovery of intestinal function, nutritional balanced intestinal nutrition preparations such as Ansol, Lishikang, etc. can be selected.

  3. Avoid (reduce) the use of foods: high-fat, spicy and刺激性 foods, such as animal fats, garlic, chili, etc.

7. Conventional methods of Western medicine for the treatment of ileocecal valve syndrome

Mild cases of ileocecal valve syndrome can usually be self-resolved without special treatment. For those with obvious symptoms, symptomatic treatment can be given. Sedatives are effective for abdominal pain, and antibacterial drugs are effective for diarrhea, especially for those with intestinal flora disorder.

If there are severe anatomical abnormalities in the ileocecal valve, or the symptoms are severe, and internal medicine treatment is ineffective, complications such as intestinal stricture, intestinal obstruction, massive rectal bleeding, or it is difficult to differentiate from acute or chronic appendicitis, surgical treatment may be considered. For prolapsed ileal mucosa, repositioning can be performed by incising the cecum. For benign small tumors limited to the ileocecal valve, they can also be removed by high-frequency electrosurgery through a fiberoptic colonoscope.
