1. What are the causes of the onset of vulvar neurodermatitis?

  1. Etiology

The etiology of vulvar neurodermatitis is not yet clear. It is generally believed to be related to dysfunction of the nervous system, imbalance in the excitation and inhibition processes of the cerebral cortex. Patients often have neurasthenia, insomnia, and climacteric symptoms. Overly tense spirit, emotional fluctuations, or excessive fatigue can trigger this disease. Scratching, friction, sweating, or other physical mechanical stimuli also often cause it. Infection and obesity can exacerbate the condition.

  2. Pathogenesis

Vulvar neurodermatitis is characterized by excessive epidermal keratinization or incomplete keratinization, with the epidermal papillae extending downward, hyperplasia and thickening of the stratum spinosum and dermis, edema within and between cells. There is chronic inflammatory cell infiltration and fibrosis around the blood vessels.

3. What are the typical symptoms of vulvar neurodermatitis?

Vulvar neurodermatitis commonly occurs at the labia majora and perineum, and can also be seen on the inner side of the thigh. The initial stage of localized neurodermatitis is characterized by intermittent local itching, which gradually worsens and becomes more itchy when exposed to heat. Due to scratching, the skin rapidly becomes lichenified, appearing as numerous millet seeds to sorghum seeds in size, pale red, brownish yellow, or round or polygonal hard and shiny flat papules consistent with the skin color, densely arranged in patches, with a surface covered with a large amount of scales. There may be scratches, blood crusts, and hyperpigmentation. The boundary of the lesion is unclear. If the course of the disease is long, the skin may become thick and rough due to long-term scratching and stimulation, showing lichenoid changes, decreased pigmentation, with the condition varying from severe to mild and recurrent, and may affect the entire body, especially the elbow pits, axilla, limbs, face, and trunk.

4. How to prevent vulvar neurodermatitis

  1, Prognosis:Recurrent attacks.

2, Prevention:Pay attention to regular life, prevent overwork, avoid washing the vulva with hot water or soap water, and avoid scratching. Avoid spicy foods.
