Prostate Ectopia

Lowley (1929) first reported prostate ectopia, proposing that the prostate tissue occurring outside the normal location of the prostate be called prostate ectopia. Prostate ectopia can occur in different locations, such as the trigone of the bladder, bladder neck, bladder wall muscle layer, base of the penis, and the distal end of the remnant urachus. Zhang Zhengwu et al. reported a giant pelvic prostate ectopia (weighing 400g), and Zhang Dingjin et al. reported a case of phimosis prostate ectopia, which is quite rare. The prostate in the bladder is often misdiagnosed as a bladder tumor.

1. What are the causes of prostate ectopia

The factors causing the development of prostate ectopia are not yet fully clear. Developmental defects caused by environmental factors during the embryonic period and sexual hormone stimulation before puberty may play a certain role in the pathogenesis of prostate ectopia.

2. What complications can prostate ectopia easily lead to

Due to the change in the normal position of the prostate tissue, it can cause a decrease in the secretion of prostatic fluid, or no secretion, a decrease in seminal discharge, and at the same time, due to its nutritional effect on sperm cells, it can also cause a decrease in sperm motility, an extension of liquefaction time, and a decrease in sperm count, etc. Therefore, clinical infertility and sterility may occur, and this disease should attract the attention of clinical doctors, and surgery should be performed as soon as possible.

3. What are the typical symptoms of prostatic ectopia

Prostatic ectopia is often manifested in clinical practice as hematuria, urinary frequency, and urinary obstruction. Butterick (1971) summarized a report of a group of 68 patients with urethral prostatic ectopia, 96.5% of whom had hematuria, followed by urinary frequency and urinary obstruction. Sometimes, blood clots can block and cause acute urinary retention. Prostatic ectopia within the pelvis usually has a long history and large size, and is often found after urinary obstruction caused by compression of the bladder or bladder neck.

4. How to prevent prostatic ectopia

The etiology of this disease is not yet clear, and it may be related to environmental factors, genetic factors, dietary factors, and emotional and nutritional factors during pregnancy. Therefore, it is impossible to directly prevent this disease based on the etiology. Early detection, early diagnosis, and early treatment are of great significance for indirect prevention of the disease. It can improve sperm quality, avoid complications such as low sperm motility and prolonged liquefaction time.

5. What kind of laboratory tests should be done for prostatic ectopia

Bladder B-ultrasound or CT scan can detect bladder filling defects. This disease is difficult to differentiate from bladder tumors, and its differentiation depends on urethral bladderoscopy and biopsy. The ectopic prostatic tissue under the microscope is smaller, often occurs singly, slightly yellow, without papillary formation, and smooth in surface.

6. Dietary taboos for prostatic ectopia patients

Prostatic ectopia should pay attention to the following two aspects in terms of diet:

  1. Pregnant women should avoid fried and roasted foods, avoid contact with radioactive sources, toxic and harmful substances, etc., in order to avoid developmental malformations in the embryonic period.

  2. Pay attention to diet and lifestyle, avoid spicy and刺激性 food, avoid smoking and drinking, avoid overexertion, in order to avoid affecting resistance.

7. Conventional Western Treatment Methods for Prostatic Ectopia

Asymptomatic prostatic ectopia can be conservatively observed without surgery. Prostatic ectopia within the urethra, due to its polypoid morphology, can be cured by simple electrocautery, with no recurrence or malignant transformation after surgery. Prostatic ectopia within the bladder can also be treated by electroresection or electrocautery; for larger prostatic ectopia within the bladder, open resection can be performed.
